2. Application Form
Before completing an application form, please read the Guidelines very carefully. Please provide concise and clear information and answer each question fully. Incomplete applications will be rejected. Applications lodged by facsimile or email will not be accepted.

Please note that P&C/P&F's are currently not eligible to apply - we recommend these groups apply to the Gambling Community Benefit Fund (refer Related Sites page for contact details).Please also note that organisations sponsoring an unincorporated group are generally not eligible, the Fund recommends that if you are unsure as to the eligibility of your organisation then you contact the secretary.
